Change of the Address of the premises specified in a licence

Should any licensed Chinese medicines traders wish to change their addresses of premises specified in their respective licences, they shall apply to the Chinese Medicines Board well in advance, and submit the application by the third day from the date of such change occurs at the latest.  Approval from the Chinese Medicines Board should be obtained prior to any change in the address of the premises. Otherwise, the licence shall become void at the expiry of 1 month from the date on which such change occurs. The licence holder shall not conduct any related business of Chinese medicines at the new premises without prior approval.

 

Application requirements

The applicant must be the licence holder or lawfully authorized person of the company; and the conditions of the new premises shall comply with the licensing requirements.

 

How to obtain application forms

Application forms and the Guidance Notes of Application for Change of Address Specified in Chinese Medicines Trader Licences can be obtained from the Chinese Medicine Division of the Department of Health or downloaded from this website. The applicants may also obtain the relevant application forms through the automatic enquiry hotline system of the Chinese Medicine Division of the Department of Health (tel.: 2574-9999)

Applicants are required to complete the Application Form for Change of Address Specified in Chinese Medicines Trader Licences (Form 12) and return them to the Chinese Medicine Division of the Department of Health in person or by registered post together with the documentary proofs of change of address of the premises.

 

Application Results and methods of payment

The applicant will receive a notification of payment if the application is approved. Upon receipt of the prescribed fee, the Department of Health may inform the applicant to bring the original licence to the Chinese Medicine Division of the Department of Health to go through necessary procedures.

For the required fees, please refer to Fees. Chinese Medicine Division will issue a General Demand Note to the applicant. The applicant could make payment according to the payment methods stated in the General Demand Note.

 

Change of other information

A retailer of Chinese herbal medicines should notify the Chinese Medicines Board in writing within 1 month from the date on which such change occurs if there is a change of the person or his deputies who is responsible for the supervision of the dispensing of Chinese herbal medicines.

A manufacturer of proprietary Chinese medicines should notify the Chinese Medicines board in writing within 1 month from the date on which such change occurs if there is a change of the person or his deputies who is responsible for the supervision of the manufacture of proprietary Chinese medicines.

Moreover, licensed Chinese medicines traders should notify the Chinese Medicines Board in writing as soon as possible for any change of other information, e.g. company's name, which has been submitted to the Board.
