Licensing of Chinese Medicine Traders > Application for renewal of licence

Licensed Chinese medicines traders may apply to the Chinese Medicines Board for renewal of licence not later than six months prior to the date of expiry.

 

Application requirements

In general, the applicant must comply with every licensing requirement and practising condition or duty of his licence, before the application for renewal is approved.

When approving a renewal, the Chinese Medicines Board may renew the licence with or without any modifications to conditions or restrictions, if any, previously imposed. The modification of such conditions and restrictions will be determined on a case by case basis.

 

How to obtain application forms

Application forms and the Guidance Notes of Application for Renewal of Chinese Medicines Trader Licences can be obtained from the Chinese Medicine Division of the Department of Health or downloaded from this website. The applicants may also obtain the relevant application forms through the automatic enquiry hotline system of the Chinese Medicine Division of the Department of Health (tel.: 2574-9999)

 

Submission of documents or information

Applicants are required to submit in person or by registered post the duly completed Application Form for Renewal of Chinese Medicines Trader Licences (Form 11) with the Documentation Checklist - Application for Renewal of Chinese Medicines Trader Licences (Checklist 11R) accompanied by all the documents listed therein. The required documents include:

  1. Copy of Business Registration Certificate;

  2. For limited company,

    1. Copy of Certificate of Incorporation; and

    2. Copy of Directors' List, such as Form AR1 of the Companies Registry or a full set of Form D1 (in case of a newly established limited company) ; OR

    For sole proprietorship, copy of Form 1(a) of Business Registration Office; OR

    For partnership, copy of Form 1(c) of Business Registration Office;

  3. Name list of sole proprietor/partners/director(s) and key personnel (including full names in both Chinese and English, Hong Kong Identity Card numbers / Passport numbers and posts. In the case of a director being a corporation, please state the name and Company Number of the corporation).

The applicant must submit the following information for the Chinese Medicines Board's assessment if the information is inconsistent with that in licence files maintained by the Board.

  1. Brief floor plan of business premises

  2. Evidence of academic qualifications and/or documentary proofs of relevant working experience of the person and his deputies responsible for the supervision of the dispensing of Chinese herbal medicines (Only applicable to retailers of Chinese herbal medicines engaging in the dispensing of Chinese herbal medicines. )

  3. List of manufacturing equipment for proprietary Chinese medicines (Only applicable to manufacturers of proprietary Chinese medicines.)

  4. Evidence of academic qualifications and /or documentary proofs of relevant working experience of the person and his deputies responsible for the supervision of the manufacture of proprietary Chinese medicines (Only applicable to manufacturers of proprietary Chinese medicines.)

 

Application Results and methods of payment

The applicant will receive a written notification if the application is approved. For the required fees, please refer to Fees. Chinese Medicine Division will issue a General Demand Note to the applicant. The applicant could make payment according to the payment methods stated in the General Demand Note.
